I believe this kind of thing occurs when there has been a TLE update between when the observation was scheduled, and when the observation occurred.

There are situations where the TLEs for a particular sat can get quite a few days out of date before they are updated by Space-Track, resulting in this kind of issue.

Looks like it is rapidly de-orbiting, according to its twitter feed.
